In
order for us to offer your child a place, we require you to read and accept our
terms & conditions listed below.
Please print out this page, sign the bottom of the
form and return it with your enrolment form and fee.
The
nursery is open through out the year except for Bank Holiday's, Easter &
Christmas. The Vestry Class 2 has two weeks at Easter four weeks in the summer
and two weeks at Christmas. Subject to change.
If a child is unwell for any reason and has to
miss sessions there will be no monetary refund. However, we do attempt to
replace sessions where possible.
Bank
Holiday sessions are charged for and can only be replaced subject to
availability. Sessions not replaced will be charged for as normal.
A
two
week deposit is payable with your 1st invoice. This will be deducted from your
last payment subject to our Terms & Conditions. Accounts unpaid will have £10
charged to the account weekly.
Six
weeks written notice must be given if your child intends to leave our nursery.
This applies in all cases without exception.
The
nursery must be notified in advance if someone other than yourself will be
collecting your child from the nursery. No child will be released unless the
collections book has been signed.
We are OFSTED inspected and approved and are
able to claim a grant towards all 3 & 4 year old children, to be eligible for
this grant a questionnaire must be completed and returned to the office. This is
subject to change at any time in accordance with the government policy.
Uniform
sweatshirts and T-shirts are available from the nursery office, they should be
worn whilst attending nursery.
Children
that are not fully toilet trained must attend nursery with at least 4 nappies
and a full change of clothes. Should we have to supply them we charge 50p per
nappy.
Cheeky
Monkeys has set limits for children. Use of physical punishment (smacking,
slapping or deprivation of food or drink) is not allowed.
All
staff have a commitment to equality of opportunity and are able to recognise and
respect differences in race, culture & religion.
Should
I/We fail to comply with these Terms & Conditions I accept that legal
action may be taken to recover any monies due, including charges for recovering
the money.
We
reserve the right to make any adjustments without notice.
Medical
You are asked on the administration form to give us
details of any serious or recurring medical problems your child may have.
We follow medical guide-lines on certain illnesses and for
anything infectious e.g. Conjunctivitis, Chickenpox, Worms, Head lice, Sickness
etc, there is a minimum period of 48 hours in which the child is not allowed to
attend nursery.
You can always phone the nursery if you are unsure. A
telephone call is also required if your child will be absent.
All medicine must be clearly labelled with your child's
name and must be handed to a member of staff. We only administer medicine on
prescription. Before any is administered, you are asked to complete a form
giving instructions with your signature.
It is essential that we have an emergency contact number
in case of accidents or illness.
Please do not send an unwell child to nursery, they will
be miserable Unwilling to learn and may pass the illness to the other children
as well as members of staff.
We do keep to the guidelines above & will send home any
unwell child.
Please note that Terms & Conditions are subject to
change. Parents of children that attend Cheeky Monkeys accept any new Terms
& Conditions form apart of our contract. January 2008 Terms & Conditions are
displayed on the notice board.
