Before you complete and submit this form we would like you to take a look at the Fee Structure, Our Terms & Conditions and our Privacy Policy before enrolling your child at our nursery. By completing and submitting this enrolment form means that you have read and understood all conditions as laid out on this website.
By
completing this form you are enabling us to meet your child's special individual
needs. Please complete this
enrolment form carefully and send it to us by pressing the submit button on the
bottom of the form. A fee of £16.00 per session, paid every fortnight is
required as well as a two week deposit. A minimum of two sessions per week must
be requested. In special cases this is negotiable. All information is treated
with the strictest of confidence and without prejudice. Please submit this form
as soon as possible to enter your child's name for possible entry into the
pre-nursery or nursery school. A £25.00 non-refundable registration fee
will be requested from you as soon as we receive this application. Your child
will then go on our waiting list for your requested intake.
Medical
You
are asked on the administration form to give us details of any serious or
recurring medical problems your child may have.
We follow medical guide-lines on certain illnesses and for anything infectious
e.g. Conjunctivitis, Chickenpox, Worms, Head lice, Sickness
etc, there is a minimum period of 48 hours in which the child is not allowed to
attend nursery.
You can always phone the nursery if you are unsure. A telephone call is also
required if your child will be absent.
All medicine must be clearly labelled with your child's name and must be handed
to a member of staff. We only administer medicine on
prescription. Before any is administered, you are asked to complete a form
giving instructions with your signature.
It is essential that we have an emergency contact number in case of accidents or
illness.
Please do not send an unwell child to nursery, they will be miserable unwilling
to learn and may pass the illness to the other children as well
as members of staff.
We
do keep to the guidelines above & will send home any unwell child.
